PRIVACY POLICY NOTICE

While the nature of insurance requires that we gather personal financial and/or health information about you, we consider the privacy of our customers a matter of great importance.  We pledge to maintain the confidentiality of that information and we do not sell, rent or give away the information you have entrusted to us.

OUR PRIVACY POLICIES AND PRACTICES

We restrict access to nonpublic personal information about you to those employees who need to know that information in order to provide products or services to you.  We maintain physical, electronic and organizational safeguards to protect information about you.

TYPES AND USE OF PERSONAL INFORMATION COLLECTED

We collect nonpublic personal information about you from the following sources:

• Information we receive from you on applications or other forms
• Information about your transactions with us, or affiliates or others
• Information we receive from a consumer reporting agency
• Information we receive from a medical information bureau

Examples of personal information we may collect from you or about you include: your name, address, social security number, telephone number and your health, financial, credit and driving history.

INFORMATION WE MAY DISCLOSE

We may disclose collected information to our affiliates and non-affiliated service providers when necessary to carry out our normal business activities. We only use and share collected information for insurance underwriting purposes, to service your policy or a claim and for other purposes required or permitted by law.

We may disclose nonpublic personal information about you, such as we have described above, to the following types of third parties that perform marketing services on our behalf or with whom we have joint marketing agreements:

• Fulfillment service providers, such as envelope stuffing services, billing and enrollment services, etc.
• Other insurance agencies or companies that perform services for us in connection with carrying out an insurance transaction involving you or your policy
• Financial institutions with whom we have joint marketing agreements, such as insurance providers
• Other types of financial institutions such as premium finance companies

You have the right to prevent us from sharing nonpublic personal information about you with a nonaffiliated third party for any purpose that is authorized by law. This is called your right to "opt out" of such information sharing.
As stated, we do not rent, sell or disclose any of your personal information, regardless of whether you are a current or former customer.
